SPORTS PHYSICAL FORMS

ALL ATHLETIC FORMS FOR THE SPRING SEASON ARE DUE TO THEIR HEAD COACH ONLY

IF THEY ARE HANDED IN AFTER THAT DATE THEY ARE CONSIDERED LATE AND THE ATHLETE

MAY NOT BE CLEARED IN TIME TO START PRACTICING.

PLEASE REMEMBER TO SIGN AND DATE ALL FORMS, INCLUDING THE HEALTH QUESTIONAIRE WHICH

IS REQUIRED TO BE COMPLETED EACH SEASON.

A PHYSICAL IS GOOD FOR 365 DAYS, SO PLEASE CHECK THE DATE OF THE LAST TIME ONE WAS GIVEN.

MEDICATION AND FORMS

Parents are to drop off all medication, along with the form signed by their doctor, to my office

 any day  from 7:30 to 3:00. All medications, including over the counter medications require a

DOCTOR SIGNATURE.

The form needed can be found below.

INJURY/CRUTCHES/CAST AND SLINGS 

Any student who needs the use of crutches, a sling, a cast, or any other medical apparatus must bring in a doctor's note to the nurse's office prior to reporting to class.

The note should include:

1. The diagnosis

2. The date the Dr. feels he/she can return to school

3. The date he/she can return to PE class and if there are any restrictions

4. Must state there is a need for crutches, how long they are to be used, if weight bearing is permitted

5. If medication is needed have the form below completed and signed by the doctor, even for over the counter medications. Then send in a bottle of the medicine needed to the nurse's office.

 6TH GRADE PARENTS 

ALL STUDENTS ENTERING 7TH GRADE ARE RECOMMENDED TO HAVE A PHYSICAL EXAMINATION BEFORE STARTING SCHOOL  IN SEPTEMBER.  FORMS CAN BE FOUND AT THE BOTTOM OF MY WEB PAGE OR AT ANY OF THE DISTRICT NURSES'S OFFICE.

WHEN COMPLETED THEY CAN BE MAILED OR DROPPED OFF AT THE HIGH SCHOOL ANY TIME.  THESE EXAMS SHOULD  BE CURRENT. THE PHYSICAL IS NEEDED TO ALLOW YOUR CHILD TO PARTICIPATE IN ALL PHYSICAL ACTIVITIES
