The following is a copy of our contract. Each student who participates in Physical Education receives a contract to bring home that is to be reviewed with parents and signed and returned to current Health/Physical Education teacher.

POLICIES AND PROCEDURES:
1.
All students are expected to be prepared for class every day.
The Physical Education school uniform is available for purchase from your Physical Education Teacher.
You may also wear a sweat suit/ warm up suit over the uniform.
Appropriate sneakers are required for class.
2. Your last name should be printed on all of your Physical Education clothes.
No other “markings” are allowed.
3.
There is to be NO jewelry of any kind worn during Physical Education classes.
(The only exception is a medical alert bracelet/necklace)
4.
No gum chewing due to safety reasons.
5.
Lockers will be assigned to every student with a school issued lock.
The locks belong to the school and will be returned to the PE department at the end of the year.
Failure to hand in the assigned lock will result in a $5.00 fine. We strongly encourage all students to lock up all of their belongings before exiting the locker room.
6.
At the conclusion of your class, you are not to leave the gymnasium or field until you are dismissed by your teacher.
7.
Conduct yourself in a mature fashion in the locker rooms.
This means no shouting, throwing of objects, or any other unacceptable behavior.
8.
Exit and return to the building via the appropriate doors. Your teacher will inform you of the proper “route” to take.
9. Do not clean sneakers off in the hallways or locker rooms.
Please remove any dirt or grass from your shoes outside of the building.
10. Failure to follow the above prescribed policies and procedures may result in other consequences and/or a lower grade.
GRADING AND EVALUATION

If you are unprepared for class, the following penalties will be in effect:
DURING ANY ONE MARKING PERIOD...
First and Second unprepared:
Zero for participation.
A notification letter will be sent home to a parent after the second unprepared.
Possible writing assignment.
Third unprepared:
Whatever letter grade you have earned to date will be lowered one (1) full letter grade.
Fourth unprepared:
Lowering one (1) additional letter grade.
Fifth unprepared
FAILURE for the marking period.
Possible elimination from Field Day.
PLEASE FAMILIARIZE YOURSELF AND YOUR CHILD WITH THESE GUIDELINES
12.
Grades are determined as follows:
25% Written tests, knowledge, rules, strategy.
50% Performance skills.
Physical tasks and daily performance.
25% Preparation, active participation, tardiness, regular attendance.
13.
Students will be evaluated from the first day of the marking period through the last day of activity in that marking period.
14.
Physical education activity excuses must be in writing from the Health Office.
If you present a parent excuse request, for full credit, you must dress for activity and then you will be sent to the Health Office. IF YOU DO NOT DRESS FOR CLASS AND YOU HAVE A PARENT NOTE, YOU WILL NOT BE DENIED THE OPPORTUNITY TO GO, HOWEVER, YOU WILL RECEIVE AN UNPREPARED.
Extended excuses must be issued from the Health Office accompanied by a physician’s written request.
At this time, you may or may not enter a study hall depending upon the number of days.
If there are areas of concern not covered on this paper, please contact your Physical Education Teacher immediately.
